A Quick Guide to Your Appointment
Before the Appointment
- You or your Primary Care Physician should call our office at (785) 537-4200. We will then need some basic information to determine which doctor and what kind of care you need. Then, we will call you (if your PCP spoke with us first) and let you know what time we have available for your appointment. Also, we will send you (or you can download) our New Patient Information Packet.
The Day of the Appointment
- All you need to do at this point is make sure that you bring everything that you need. This includes...
- Any other tests or records pertaining to the problem area. If you aren't bringing these, make sure your Primary Care Physician sends them over before the appointment.
- Any necessary forms.
- Complete insurance information
- Any completed paperwork we may have mailed you (patient forms)
- If under you are 18 years of age, a parent/guardian will need to accompany you unless prior arrangements have been made.
