A Quick Guide to Your Appointment

Before the Appointment

The Day of the Appointment

  1. Any other tests or records pertaining to the problem area. If you aren't bringing these, make sure your Primary Care Physician sends them over before the appointment.
  2. Insurance Card(s)
  3. Insurance Copay (if applicable)
  4. Any completed paperwork we may have mailed you (patient forms)
  5. If you are under 18 years of age, a parent/guardian will need to accompany you.
