A Quick Guide to Your Appointment

Before the Appointment

• You or your Primary Care Physician should call our office at (785) 537-4200. We will need some basic information from you. We will send you (or you can download) our New Patient Information Packet.

The Day of the Appointment

• Please bring the following with you:

1. Any other tests or records pertaining to the problem area. If you aren't bringing these, make sure your Primary Care Physician sends them over before the appointment.
2. Insurance Card(s)
3. Insurance Copay (if applicable)
4. Any completed paperwork we may have mailed you (patient forms)
5. If you are under 18 years of age, a parent/guardian will need to accompany you.
