Patient forms
What forms you will need depends upon your injury
and time since you were last at our clinic.
If you are a new patient, you'll need our New Patient
Information Packet, as well as one of the following letters (listed by the clinic at which you will be seen):
- Orthopaedic and Sports Medicine Center (Manhattan)
- OSMC (Junction City)
- Clay Center
- Marysville
- Council Grove
- Washington
If you are a returning patient, and it has been over a
year since we've last seen you, you'll need to fill out
a new Health History form.
Also, if you had a new injury since you were
last seen, you'll need our Reason for Visit form.
